IQStep2 CK was built because existing QBanks weren't written for how IMGs think — they were written for US medical students with a different clinical training context. We fixed that.

The authoring process

How questions are written

Every question starts as a real clinical vignette archetype drawn from USMLE Step 2 CK blueprints and recent exam reports. Each stem is then reviewed by at least two physicians — one US-trained, one IMG — to flag assumptions that don't hold outside the US training context.

Questions go through three editorial passes: factual accuracy, distractor plausibility, and IMG relevance. A question only ships when every distractor is defensibly wrong and the stem doesn't rely on knowledge an IMG graduate is unlikely to have encountered in clinical training.

The explanation philosophy

Why 7 layers?

Most QBank explanations tell you what the right answer is. They rarely explain why you picked the wrong one — which is the only information that changes behavior on the next question.

Our 7-layer format (quick answer → correct-choice rationale → distractor-by-distractor review → exam clue → common trap → takeaway → IMG context) is designed so that a learner who reads every layer once is more likely to get the next similar question right than a learner who re-reads the stem five times.

Editorial standards

What we check before a question ships

Factual accuracy

Every clinical fact is cross-referenced against UpToDate, USPSTF, and current ACOG/ACC/AHA guidelines at the time of authoring. Questions are retired when guidelines change.

Distractor plausibility

Every wrong answer must represent a common real-world reasoning error — a trap that reflects a genuine clinical decision branch, not a random wrong fact.

IMG relevance

Each question is reviewed for assumptions about clinical exposure (e.g., pelvic exams, obstetric emergencies, psychiatric holds) that differ between US and international training.

Stem clarity

We run every stem through a readability pass to remove ambiguous pronouns, double-barreled questions, and 'EXCEPT' constructions that test reading comprehension instead of clinical reasoning.

The team

Built by IMGs, reviewed by physicians

The core team includes physicians who matched into US residency programs as IMGs and medical educators with experience in USMLE prep curriculum. We don't publish names to avoid the impression that any single physician's clinical opinion is the standard — USMLE Step 2 CK tests guidelines and best-evidence practice, not individual preference.

Questions with contested answer keys or recent guideline changes are flagged for review within 30 days of the guideline update. If you believe a question has an error, use the flag button during any quiz — every report is reviewed within 5 business days.
